Attention Licensees: For all new applications or renewal applications, please use the online application links provided below. Should a paper copy of an application be required, please contact the Board office during normal business hours or send a request to boardofpharmacy@wv.gov. Permits and licenses will not be printed and mailed. Applicants/Licensees will receive an emailed link to print the permit/license. The link will be sent to the email which is on file with the permit/license. If a printed and mailed copy is required, please click appropaite link below.
