WV Board approval number for Drug Diversion CE will be required during renewal.  If you do not have the WV approval number please see list of WV Approved Drug Diversion.

§15-1-2.1.40 “Pharmacist” means an individual currently licensed by this state to engage in the practice of pharmacist care.

Renewal must be complete and include a $120.00 registration renewal fee payable to the West Virginia Board of Pharmacy.  Applications received in the Board office after June 30 requires payment of a late fee of $100.00 in addition to your regular renewal fee, to total $220.00.  If you fail to submit your application for renewal by August 31, your authorization will be considered lapsed.  If lapsed, you will be required to apply for reinstatement using the appropriate reinstatement form. 

In order to renew your Pharmacist Registration, you must meet the following requirements:

  • Per WV Code §30-1-20 Anticipated Retirement Date is required
  • WV Residents require 30 hours continuing education.  Of that 30 hours, 6 hours must be Live and 2 hours must be a WV Board of Pharmacy approved Drug Diversion unless the Drug Diversion waiver is signed
  • Non-residents require certification that continuing education requirements are being met in the licensee’s home state.  In addition, 2 hours of a WV Board of Pharmacy approved Drug Diversion must be completed unless the Drug Diversion waiver is signed
  • All licensees are required to be registered with and maintain access to the WV Controlled Substance Monitoring Program unless waiver is signed.

Reinstatement

15-1-5.5.4.  In order for any pharmacist, pharmacy intern, or pharmacy technician whose name has been removed from the register of the board to again become licensed or registered, the pharmacist, pharmacy intern or pharmacy technician shall petition the board, or an authorized committee of the board, for reinstatement, in writing, to show cause for permitting the license or registration to lapse.  If his or her license or registration has been expired for one year or less (i.e., the petition for reinstatement is received on or before June 30 of the year after his or her authorization expired), and if the board finds the person otherwise eligible and qualified to practice, the Board shall reinstate that person upon payment of reinstatement fee of $250.00 for a pharmacist plus the renewal fee of $100.00, or upon payment of a reinstatement fee of $50.00 for a pharmacy technician plus the renewal fee of $20.00.  If the pharmacist license or pharmacy technician registration has been expired for more than one year (i.e., the petition is received after June 30 of the year after his or her authorization expired), the board finds the person has submitted to the board satisfactory reasons for allowing the license or registration to lapse, and satisfies the board as to his or her qualifications to practice the profession by successfully passing the examinations administered or otherwise required by the board for reinstatement, the Board shall reinstate that person upon payment of reinstatement fee of $250.00 for a pharmacist plus the renewal fee of $100.00, or upon payment of a reinstatement fee of $50.00 for a pharmacy technician plus the renewal fee of $20.00.  If a pharmacy intern’s license has been expired for more than a year, he or she must make new application as an intern and pay the required application fee for an initial pharmacy intern license.

Pharmacist less than 1 year:

  • Complete reinstatement form

  • Provide proof of any required CE

  • Pay reinstatement fee of $250.00, renewal fee of $100.00 and PRN of $20.00, to Total $370.00

Pharmacist more than 1 year:

  • Complete reinstatement form

  • Provide proof from national board of registration for MPJE

  • Pay reinstatement fee of $250.00, renewal fee of $100.00 and PRN of $20.00, to Total $370.00
