5/28/2024: Pharmacies report a phoned in prescription for a Z-Pak and Promethazine syrup. They provided proper name, NPI, 304-726-4860, Jeffrey T. Cook from One Medical Choice in Keyser, WV. Similar call to another pharmacy under provider John Cook.  This is almost identical to some from about 10 days ago. Please be alert.
